    This paper includes, as an annex, Law No. 7693, "On Urban Planning," from the People's Assembly of the Republic of Albania. Conceptually, this law has five major parts: (1) planning generally, (2) getting construction permission, (3) special provisions for tourist zones, (4) special provisions for military zones and zones with singular (that is, archaeological, historical, or cultural) value, and (5) penalties for violations.     Insurance and the Utilization of Medical Services Among the Self-Employed

    There has been substantial public policy concern over the relatively low rates of health insurance coverage among the self-employed in the United States. We use data from the Medical Expenditure Panel Survey conducted in 1996 to analyze how the self-employed and wage-earners differ both with respect to insurance coverage and utilization of a variety of health care services. Our results suggest that for the self-employed, the link between insurance and utilization of health care services is not as strong as assumed in the policy debate. For a number of medical care services, the self-employed have the same rates of utilization as wage-earners, despite the fact that they are substantially less likely to be insured. And when the self-employed are less likely than wage-earners to utilize a particular medical service, the differences generally do not seem very large. The self-employed thus appear to be able to finance access to health care from sources other than insurance. Further, analysis of out-of-pocket expenditures on health care suggests that doing so does not lead to substantial reductions in their ability to consume other goods and services. Finally, there is no evidence that children of the self-employed have less access to health care than the children of wage-earners. Hence, the public policy concerns that the relative lack of health insurance among the self-employed substantially reduces utilization of health care services or creates economic hardship appear to be misplaced.

    Effective Crew Operations: An Analysis of Technologies for Improving Crew Activities and Medical Procedures

    NASA's vision for space exploration (February 2004) calls for development of a new crew exploration vehicle, sustained lunar operations, and human exploration of Mars. To meet the challenges of planned sustained operations as well as the limited communications between Earth and the crew (e.g., Mars exploration), many systems will require crews to operate in an autonomous environment. It has been estimated that once every 2.4 years a major medical issue will occur while in space. NASA's future travels, especially to Mars, will begin to push this timeframe. Therefore, now is the time for investigating technologies and systems that will support crews in these environments. Therefore, this summer two studies were conducted to evaluate the technology and systems that may be used by crews in future missions. The first study evaluated three commercial Indoor Positioning Systems (IPS) (Versus, Ekahau, and Radianse) that can track equipment and people within a facility. While similar to Global Positioning Systems (GPS), the specific technology used is different. Several conclusions can be drawn from the evaluation conducted, but in summary it is clear that none of the systems provides a complete solution in meeting the tracking and technology integration requirements of NASA. From a functional performance (e.g., system meets user needs) evaluation perspective, Versus performed fairly well on all performance measures as compared to Ekahau and Radianse. However, the system only provides tracking at the room level. Thus, Versus does not provide the level of fidelity required for tracking assets or people for NASA requirements. From an engineering implementation perspective, Ekahau is far simpler to implement that the other two systems because of its wi-fi design (e.g., no required runs of cable). By looking at these two perspectives, one finds there was no clear system that met NASA requirements. Thus it would be premature to suggest that any of these systems are ready for implementation and further study is required

    Child criminal exploitation : a new systemic model to improve professional assessment, investigation and intervention

    Child Criminal Exploitation (CCE) means coercive control of a child in order for them to commit crimes on the behalf of another. Often, a child that is a victim of criminal exploitation is not recognised as such when they come to the attention of criminal justice and child safeguarding authorities.This thesis challenges the assumptions which underpin the tactical and strategic responses of professionals, and criminal justice and child safeguarding organisations. It provides a new theoretical model, which could be used for extending and improving the range of responses to the problem by analysing it as a pattern that emerges from the interactions between the child, the perpetrators of the exploitation and the environments in which they both exist.Drawing upon diverse extant theories to generate an emergent, constructivist model CCE (which I have called “The Circles of Analysis”), the questions that are addressed in this thesis are: What are the components and mechanisms that maintain the relationships between the child, their environment and the perpetrators that lead to criminal exploitation? Precisely how can the relational understanding of child criminal exploitation, proposed in this thesis, result in better strategic and tactical responses by organisations concerned with safeguarding children and pursuing and prosecuting offenders? Can the theoretical model for CCE proposed in this thesis support and improve professional judgement and decision making in relation to safeguarding children at risk of criminal exploitation?Through novel use of focus groups as a method of “market testing” the viability of the Circles of Analysis concept with practitioners, the final iteration of the model was coproduced with research participants. The result is a model that reflects professionals’ experience of CCE and has practical applications. It supports identification and analysis of CCE, aids professional judgement and decision making and can improve collaborative safeguarding interventions

    CropEx Web-Based Agricultural Monitoring and Decision Support

    CropEx is a Web-based agricultural Decision Support System (DSS) that monitors changes in crop health over time. It is designed to be used by a wide range of both public and private organizations, including individual producers and regional government offices with a vested interest in tracking vegetation health. The database and data management system automatically retrieve and ingest data for the area of interest. Another stores results of the processing and supports the DSS. The processing engine will allow server-side analysis of imagery with support for image sub-setting and a set of core raster operations for image classification, creation of vegetation indices, and change detection. The system includes the Web-based (CropEx) interface, data ingestion system, server-side processing engine, and a database processing engine. It contains a Web-based interface that has multi-tiered security profiles for multiple users. The interface provides the ability to identify areas of interest to specific users, user profiles, and methods of processing and data types for selected or created areas of interest. A compilation of programs is used to ingest available data into the system, classify that data, profile that data for quality, and make data available for the processing engine immediately upon the data s availability to the system (near real time). The processing engine consists of methods and algorithms used to process the data in a real-time fashion without copying, storing, or moving the raw data. The engine makes results available to the database processing engine for storage and further manipulation. The database processing engine ingests data from the image processing engine, distills those results into numerical indices, and stores each index for an area of interest. This process happens each time new data is ingested and processed for the area of interest, and upon subsequent database entries, the database processing engine qualifies each value for each area of interest and conducts a logical processing of results indicating when and where thresholds are exceeded. Reports are provided at regular, operator-determined intervals that include variances from thresholds and links to view raw data for verification, if necessary. The technology and method of development allow the code base to easily be modified for varied use in the real-time and near-real-time processing environments. In addition, the final product will be demonstrated as a means for rapid draft assessment of imagery

    Translation research: from accurate diagnosis to appropriate treatment

    This review article focuses on the various aspects of translational research, where research on human subjects can ultimately enhance the diagnosis and treatment of future patients. While we will use specific examples relating to the asbestos related cancer mesothelioma, it should be stressed that the general approach outlined throughout this review is readily applicable to other diseases with an underlying molecular basis. Through the integration of molecular-based technologies, systematic tissue procurement and medical informatics, we now have the ability to identify clinically applicable "genotype"-"phenotype" associations across cohorts of patients that can rapidly be translated into useful diagnostic and treatment strategies. This review will touch on the various steps in the translational pipeline, and highlight some of the most essential elements as well as possible roadblocks that can impact success of the program. Critical issues with regard to Institutional Review Board (IRB) and Health Insurance Portability and Accountability Act (HIPAA) compliance, data standardization, sample procurement, quality control (QC), quality assurance (QA), data analysis, preclinical models and clinical trials are addressed. The various facets of the translational pipeline have been incorporated into a fully integrated computational system, appropriately named Dx2Tx. This system readily allows for the identification of new diagnostic tests, the discovery of biomarkers and drugable targets, and prediction of optimal treatments based upon the underlying molecular basis of the disease

    Decision Support for Emergency Operations Centers

    The Flood Disaster Mitigation Decision Support System (DSS) is a computerized information system that allows regional emergency-operations government officials to make decisions regarding the dispatch of resources in response to flooding. The DSS implements a real-time model of inundation utilizing recently acquired lidar elevation data as well as real-time data from flood gauges, and other instruments within and upstream of an area that is or could become flooded. The DSS information is updated as new data become available. The model generates realtime maps of flooded areas and predicts flood crests at specified locations. The inundation maps are overlaid with information on population densities, property values, hazardous materials, evacuation routes, official contact information, and other information needed for emergency response. The program maintains a database and a Web portal through which real-time data from instrumentation are gathered into the database. Also included in the database is a geographic information system, from which the program obtains the overlay data for areas of interest as needed. The portal makes some portions of the database accessible to the public. Access to other portions of the database is restricted to government officials according to various levels of authorization. The Flood Disaster Mitigation DSS has been integrated into a larger DSS named REACT (Real-time Emergency Action Coordination Tool), which also provides emergency operations managers with data for any type of impact area such as floods, fires, bom

    The Self-Employed are Less Likely to Have Health Insurance Than Wage Earners. So What?

    There is considerable public policy concern over the relatively low rates of health insurance coverage among the self-employed in the United States. Presumably, the reason for the concern is that their low rates of insurance lead to worse health outcomes. We use data from the Medical Expenditure Panel Survey conducted in 1996 to analyze how the self-employed and wage-earners differ with respect to insurance coverage and health status. Using a variety of ways to measure health status, we find that the relative lack of health insurance among the self-employed does not affect their health. For virtually every subjective and objective measure of health status, the self-employed and wage earners are statistically indistinguishable from each other. Further, we present some evidence that this phenomenon is not due to the fact that individuals who select into self-employment are healthier than wage-earners, ceteris paribus. Thus, the public policy concern with the relative lack of health insurance among the self-employed may be somewhat misplaced.
